PROVING VACCINES RECEIVED IN UKRAINE

How do I prove that my child was vaccinated in Ukraine while I am abroad in the European Union?

Children abroad are required to show proof of vaccination in order to enroll in school.


The method for confirming vaccinations received in Ukraine while abroad has been simplified, with relevant guidelines available for European healthcare workers.

   To confirm you or your child’s vaccine status in the European Union, present one of the following forms:    

  • History of child development (form No. 112/o)  
  •    
  • Card of preventive vaccinations (form No. 063/o)  
  •    
  • Medical card of an outpatient patient (form No. 025/о)
  •    
  • International Certificate of Vaccination
  •    
  • Or Certificate of Immunizations  


  • What to do if your child is vaccinated but you cannot prove it

    If you have vaccinated your child but cannot prove that your child has been vaccinated with all the recommended vaccines, you may be required to vaccinate them again. In this case, do not worry. It is safe to repeat the vaccines that have already been administered.
